University of AZ Health Plan

In February 2015, the Health Plans merged with Banner Health. As such, UAHP operates as one component of an integrated health care system that includes Banner University Medical Center Tucson Campus and South Campus as well as a comprehensive network of Banner Health primary care and specialty care providers. We also have a robust and diversified community provider network across all counties of operation. Our goal is to ensure that our members have access to care nearby and that primary care providers have a good selection of local providers with which to work and refer our members.

University Family Care (UFC).

University Family Care (UFC) is offered to Arizona Health Care Cost Containment System (AHCCCS), eligible members. Eligibility is determined by the Arizona Department of Economic Security (ADES) and the AHCCCS Administration.

University Care Advantage (HMO SNP)

University Care Advantage (UCA) is a Dual Eligible Special Needs Plan (D-SNP) members with both Medicare and Medicaid. Members must be entitled to Medicare Part A, enrolled in Medicare B and AHCCCS and reside in a contracted service area in order to be eligible.

University Care Advantage is a Medicare Advantage HMO SNP offered by University Care Advantage Inc., with a Medicare contract and a contract with Arizona Medicaid program. Enrollment in the plan depends on contract renewal.
Medicare Advantage Special Needs Plan available to individuals who are entitled to Medicare Part A, enrolled in Medicare Part B and AHCCCS (State Medicaid program) and live in the service areas of Cochise, Gila, Graham, Greenlee, La Paz, Maricopa, Pima, Pinal, Santa Cruz, Yavapai and Yuma Counties in Arizona
To join University Care Advantage (HMO SNP), you must be entitled to Medicare Part A, be enrolled in Medicare Part B and the Arizona Health Care Cost Containment System or AHCCCS (Medicaid), and live in our service area. The service area includes the following county in Arizona: Cochise, Gila, Graham, Greenlee, La Paz, Maricopa, Pima, Pinal, Santa Cruz, Yavapai, and Yuma 

WHICH DOCTORS, HOSPITALS, AND PHARMACIES CAN I USE? University Care Advantage (HMO SNP) has a network of doctors, hospitals, pharmacies, and other providers. If you use the providers that are not in the network, the plan may not pay for these services. You must generally use network pharmacies to fill your prescriptions for covered Part D drugs. WHAT DO THEY COVER? Like all Medicare health plans, The plan covers everything that Original Medicare covers– and more. • Plan members get all of the benefits covered by Original Medicare. • Plan members also get more than what is covered by Original Medicare.
